App to manage insurance denials?

I'm trying to find an app that can handle the following:

1. Web Based (cloud)
2. Accept a contact list of customers (insurance companies in our case)
2. Can enter bills that were denied/not paid by these customers for multiple locations/clinics
3. Can allow uploading of scanned documents for each bill denied
4. Can enter notes for calls and other work done to resolve the denial with customer
5. Can generate an aging report of total balance outstanding in denials by location
6. A smartphone interface would be icing
